How to schedule an appointment

Use the form to submit your requested appointment time. If you don’t see a time that works for you, don’t worry! We typically have a 24-48 hour turn-around for appointment availability.

Note: Your appointment is not confirmed until you hear from our staff by phone, email, and/or text. 

After you have scheduled your appointment, don’t forget to complete your Patient Information (link below). This gives us everything we need before your appointment so you don’t have to waste time in the clinic completing your information.
